However, previous studies have demonstrated that accurate and timely collection of patients’ medication histories is challenging especially in the ED for various reasons, including patients with altered mental status due to confusion or intoxication, patients taking multiple outpatient prescriptions, and first-time patients to the hospital [14-16]. Since the treatment plan would change depending on the medication history, the prompt and complete evaluation of the medication history is vital.
