To define the interrater reliability of the coding template, we calculated Fleiss’ Kappa using R (version 4.0.5; R Foundation for Statistical Computing). We calculated every statement's median and IQR for the survey analysis. We used the Wilcoxon signed rank test to determine the difference between the median and neutral answers. We used R version 4.0.5 (R Foundation for Statistical Computing,) to analyze all data and used Prism 9 (Graph Pad Software Inc) to create all figures. We considered a P-value of less than .05 to determine statistical significance.
In May 2020, we recruited 52 teams performing 156 simulations. We excluded 12 teams as the eye-tracking setup was revised because the laminated QR codes used reflected and were not detected by the eye-tracking software.

To quantify the interrater reliability, when assigning the individual statements to a particular topic of the final coding tree, we calculated the Cohen kappa using R, version 4.0.5 (R Foundation for Statistical Computing) [19]. To estimate percent agreement, we used Microsoft Excel.
We acquired field notes between May 4, 2020, and May 28, 2020. All participants were employees of the University Hospital of Zurich.
