One trial reported statistically significant effects at 6-month follow-up (P=.03), but these effects were not sustained at 12 months (P=.62) [46]. This null result at longer follow-up may be explained by the use of atheoretical, simple text prompts. Although simple prompts may be effective for short-term behavior change, they may create reliance on the text message as a prompt. In turn, behavior change may not be sustained upon message cessation.

In a randomized controlled trial (RCT) in 21 primary care practices in England, He LP-Diabetes led to a significant, albeit modest, reduction in Hb A1c levels of 0.24% (95% CI −0.44% to –0.049%; P=.01) at 12 months and was found to be cost-effective [14]. The He LP-Diabetes website contained information about understanding and treating T2 DM, behavior change modules, self-help tools, self-assessment quizzes, videos from people with T2 DM, a moderated web-based forum, and an electronic health record.
